• Manage your prescription refills. View your medication history.

• Overdue for a checkup or a vaccine? MyChart will recommend preventative care for you!

• View visit summaries, test results, discharge instructions, and more.

• Pay your bill. View your payment history.

• Save time by checking in before you arrive at any CommWell Health location.

How to start your CommWell Health MyChart account:

On your smartphone:

Go to your app store (iPhone or Android) & search for “MyChart”. Download & install the MyChart app on your phone

On your desktop computer/laptop/tablet:

Type this address into your browser address bar: https://mychart.hcnetwork.org/MyChart/Authentication/Login?

At a CommWell Health practice location:

A CommWell Health colleague can assist you with signing up for MyChart while you’re at the front desk, in the exam room, or at discharge.

• Ask a Guest Services Associate (Front Desk) for an activation email sent to your personal email address. Click the link in the email to proceed.
• Create your login ID and password, enter your DOB, and click CONTINUE.
• Scroll through the list of healthcare organizations and select CommWell Health.
